Dear Patient

Please be advised that because of my retirement I am discontinuing the practice of Orthopaedic Surgery next month, on December 22, 2023.  I will be unable to continue your care or renew your prescriptions after that date. 

I suggest you place yourself in the care of another physician.  If you are not acquainted with another Orthopaedic Surgeon, I suggest that you contact Joseph Nelson, M.D. with Dignity Health Medical Group Orthopaedics.   The Primary Care Physician that referred you to my practice is also a very valuable resource. 

Records will be stored securely according to Federal and State confidentiality laws and protected until 2030.  I have retained Desert River Solutions  as Custodian of your Medical Records.   

I am sorry that I cannot continue as your Orthopaedic Surgeon.  I extend to you my best wishes for your future health and happiness.
